Do not run that engine anymore.
It is done, Id bet any money on that.
Any more running the engine, and its very possible you could see internals on your garage floor VIA the oil pan.

Kept an eye on all fluid levels diligently. What is going on?

Something was VERY close to going inside of your engine, in other words, a pre-existing problem, and the added air/fuel aggrivated it and sped up the process. This is just from what I can tell. Something went wrong for it to go like that.
You can remove the oil pan if you wish, It wont fix anything at all at this point from the sound of it, but it cant hurt to see some hard evidence of whats really happening in there.
Could even be some material on the bottom of the oil pan as well.
You may be in the market for a compleat engine used or new...

Seems like somehow coolant got in there which is a huge shame since you replaced the UIM to prevent this from happening. :( I'm assuming you torqued everything properly? I'm sorry this happened!

I doubt coolant got in there. Won't know until I tear it down though. Everything was torqued properly. I followed all SOP'*. This sucks.........
