UPDATE:
Failure analysis: 2nd gear clutches nearly burnt up. Some damage to second gear, but not major. Major failure was a broken pin in the diff. The pin between the two spider gears. Pin half backed out, and wore a groove in the housing until it was paper-thin and cracked. Don (trans builder) has never seen a failure like this in 15 years of trans rebuilding. He was impressed with the overall condition of the rest of trans, and wondered why it was so clean considering. I told him a flush a year, and he told me to keep it up.
