If the fans are on constantly even when cold I would suggest a new thermostat/coolant temp sensor.

My alero was different the temp would only read a 1/4 and fans were on constant on high. I replaced both thermo and temp sensor and problems were gone.

That should hopefully fix your heat issue.

It looks like it'* all related to the coolant. Once I flushed everything out and put the apropriate amount of coolant, the rough idle went with it.
