Here'* my card in Word format. I can convert it to just about anything anyone might need, so just PM me if you do.

Feel free to 'adjust' the format/content to suit your needs. I print both pages, then flip the paper back and print two more. Front/back of the cards on 2 sheets, then reverse them.

Heavy card stock works great for the paper:

I heard from Aviator that my file was 0kb in size. I just fixed it. If it asks for a password, just hit esc. This is Word 2000 Pro format. Be warned, the formatting is slightly off, so there is a left to right offset between the front and back of the card. Not much, but I noticed it when I cut them.
