91 Buick Regal Stalls when Starting

91 Buick Regal Stalls when Starting
Friends series1 3800=Problem sometimes when first starting cold or after sitting an hour lately, it stalls out and requires 3-4 re starts before it will run normal while applying gas. no codes or CE light. Engine seems to run good after that.
I asked him to temp use higher octane fuel, and I put a bottle of water-remover in fuel & carb/injector cleaner in new 94 oct fuel.
My guess is to start at fuel filter with replacement. May not have been replaced on this car in a while as it has about 250k miles and he has had it less that 1 year.
What about fuel pressure regulator could this be causing a problem too?
Unfortunately I was not able to remove the IAC today as it is angled on 45 degree and someone has changed one of the Phillips screws with a T-20, and I have no allen type t-20 to get in there.

When the stalling issue is to expected, hold the throttle open a tad, then start it. If it starts immediately, and you have to hold the throttle open a tad(and I mean a TAD) to keep it going, then the IAC is not opening like it should.

Mike thanks for that reply. Going to remove the IAC somehow. Probably going to have to wait until my buds shop is open again in the new year and see what he has in his shop tools to access the darn 45 degree angled IAC. I used a 1/4 ratchet / socket/T-20 bit and could not get the right angle on it without removing the thermostat...geeze my 07 PA was so easy to access the IAC.
