What car are we talking about here? What I did in my Bonneville is I picked up one of these:

...and plugged it into the back of the stock head unit. Then I got an RCA to 3.5mm cable to plug the mp3 player into it. I just turn the radio on and press "play" on the tape deck and it works.
I also have a noise reducer that cancels out the noise I was hearing in between songs:

if your brave and cheap and handy with a soldering iron, there have been people who wire in one internal to the radio.

my sis uses something for the tape deck. idk what its called...but she inserts this tape into the tapeplayer of the stock HU and out comes a 3.5mm wire. she uses it for her ipod

my dad got some thing where its an adapter that is put into his stock head unit in his 02 lesabre through the back, an ipod plug comes out the bottom and he just plugs in his ipod and it plays through i believe the radio. basically a high tech radio transmitter connected directly into the head unit
