Well, I found at least part of the problem. I used a vacuum gun to test the boost control actuator (BCA), and it didn't hold pressure so I replaced it with one that did hold pressure. There must have been a small vacuum leak for quite some time - maybe years!
The idle is smoother, but still not quite as smooth as I'd like.
