Why you should use jack stands
I have been working on replacing a power steering line in a 95 Z34 for a few days now. Got it done tonight, but not without learning a lesson. I have had the car up on a descent floor jack for about 3 days now so I could work underneth it (where I spent most my time). I had done some top side work and was debating going back underneth to loosen a bolt to see if I could get more line play. As I am looking at the car contimplating it I see my jack stand go and the car starts jolting down on the passenger side (the side with a jack only, no jack stand). Looking at where I would have been just moments later I realized if I had been there, I would have been crushed by the front k-frame from my waste up. So moral of the story is, if you plan on going under your car, make damn sure you have jack stands and not just the jack.
Every now and then I'll go under without jack stands... but this is with two of my floor jacks supporting the same/similar spot. One gives out, the other'* still holding. But this is only for quickies like oil changes.

I've been close to having that happen.
Generally it'* my jack tipping over, and I've always been able to kick it the right direction and save it.....Keep in mind anytime I've done that there'* always been a tire on the car.
