cause most 4 cylinders are an inline design and not in a V

It refers to the configuration of the engine. V engines have two rows of cylinders forming a V shape. As opposed to an inline engine it takes up much less space.

There is a V4 engine in Honda motorcycles. VFR700, 750 and 800 are among some of the model that use the V4 configuration

If you look at whatever side of the engine the drive shaft comes out of you will notice that the cylinders are arranged in a V. 4 cylinders are usually inline, meaning all four cylinders are lined up in a row. The trucks you drive are most likely inline sixes with cylinders that have a diameter of a basketball.

and the V is the designated design of the block the cylinders are in a V formation.....like wise the Inline engines have the cylinders are in an inline formation

There are many ways engines are laid out. V, I, F, and even W. F (Flat) engines are used in Porsches, and VW uses a W12 in some cars.
