ive gotten that a few times. i just hit F5 and then it will do what it was suposta do. sometime puters are just stupid.

The form is based on PHP and SQL. For whatever reason, there was a hiccup and the software tried to insert an entry into the database more than one time. It'* a standard SQL error message with a standard SQL query to the server.
