It refers to the configuration of the engine. V engines have two rows of cylinders forming a V shape. As opposed to an inline engine it takes up much less space.

If you look at whatever side of the engine the drive shaft comes out of you will notice that the cylinders are arranged in a V. 4 cylinders are usually inline, meaning all four cylinders are lined up in a row. The trucks you drive are most likely inline sixes with cylinders that have a diameter of a basketball.
