I had a VERY similar situation on a 94 Olds. It ended up being the harmonic balancer of all things. All symptoms were the same as yours, and I did all of the same troubleshooting steps...
Check your HB for cracks in the rubber around the rim. Age may have gotten the best of it.

well the car was back firing, when runing cold,, fixed,,, after i swiched out plugs and wires it was still runing bad, so i started pushing on the wires to make sure thay where on all the way,, and when i was pushing on the 1st coil it ran good, and when i let go it ran bad,. So i pulled it and swiched it, SAME,, next i cleaned the ICM and put some of that gel,on the ICM and i runs good now,
it seems that the inside on the coil wasnt makeing good contact with the ICM
It still has a stumble at rev up, when i first punch it, but it allwas did that
Sorry if im going off topic, ""has had a small wine at low speeds, but shifts good,""
i know its not good, i just dont know how bad, ive had the car for 4 1/2 years and it always had a wine, never got better or worse
sorry i have many questions just dont want to post tomany topics,
