Default '02 SE with really bad shimmy

I've developed a horrible shimmy between 65 and 75 mph. I don't necessarily feel it really bad in the steering wheel, but it does make the entire dash hop. I decided to do a tire rotation today and found the driver'* side rear was worn to the cords on the outside and diagonal sections across the tread plate all the way around the tire that was worn almost bald. The front tires looked good. The passenger'* side rear was worn a little more around the outside than the inside, but really not that bad. Any ideas on what the heck may have caused this? I have replaced the ?tracking bar? thing in the rear on the passenger because the ball joint on the end came apart. I've been thinking the driver'* side one should be done to, but it really doesn't feel that bad. I never did bring it in for an alignment after I changed that and did notice that it does have an adjustment.
