Default KVM Switch

Anyone know where I can get a 4-Port KVM switch that supports USB in and out and has DVI conections, between the price of 100-200?

As in 4 computers to hook up, or 4 ports total, or 4 ports for 4 computers? Or something else....?

4 computers (keyboard, video, mouse) to control from one source. Each port on the switch must contain a DVI video plug instead of VGA.
