Print it out on Avery label paper actual size. Trim and stick. I use full-sheet Avery paper, or the Staples generic brand. I simply did a screen capture of this on Ebay, and a cut and paste, then crop in PS. There'* so many of these being sold, I figure the seller did the same. And there were no copyright or trademark warnings listed on the auction
