Gas Milage Database... Need info!

I am looking to see what the trends and differences are on certain items that affect gas milage on our cars

So i am going to put a database together and figure out what helps and hurts etc. that we might normally overlook and how much. So what i will need from everyone of you that measures your milage is to fill out as many of these as you can:

1) Year and Type of car (N/A / Super)
2) Milage on car
3) Octane Rating used regularly
4) Type of spark plug
5) CAI (Yes/No)
6) Free-flow muffler/ exhaust (Yes/No)
7) Injector Cleaner Usage
8)Top Clean/Seafoam (Yes/No)
9) MPG or Miles per tank (and gallons used if known)
10) Short Description of your driving style

Thank you all, i appologise for the length of it... However, the more of you that respond the better ideas i can get and report back!
