I'm at a loss here, freak. How did you remove the cig lighter to begin with? Shouldn't it snap back in? Missing retainer?

i had the same problem with m ine, and i replace the whole rear part of the console.. If you look carefully, the vinyl covering is probably not seated on the plastic framing anymore. when i replaced mine, me and billboost noticed this. the vinyl starts to separate and wont wrap on and hold tigh to allow the cklearance necessary for the lighter. ill see if i can take some pictures to show you exactly what im talking about, if i can find the old part

the reason for the one notch is that you put the right side in first, use the notch to put the left side in, then twist to lock in place

I think you are looking at the same issue as Toasted, The vinyl is fighting you.

Be careful, your fingers are not intended to go in the cig socket.

i talked to andrew on the phone, and i dont think this is his problem, although it is still very possible.. i really cant describe that well over the phone exactly what the issue was, and it should be looked into a bit closer.
