Default shreading belts

i have a question,what would cause a belt to keep gettin basically sliced on the left side

In my past experience, it'* always been a bent or misaligned pulley. I've also seen improper reinstallation of bolts i.e. water pump bolts that the heads were rubbing against the belt(*)
