Default 1993 reverse reaction shell chewed up

The car drove fine no signs of begining failure has 143,000 miles 1993 3.8 engine non-supercharged. One day driving bang trans goes. I had all drive gears to about 15mph and no reverse. I pulled engine and trans together out. Took trans apart. Found the reverse reaction shell bad and some kind of spacer. I can get the shell for $28.00. I don't know what the spacer is called it is made of plastic about 3" i.d. 4" o.d. One other spacer too is bad i had to break to get shaft out. It is plastic tight around shaft and has a 1/2 inch flange all around it.

Can some one help on the parts

what may have caused this prob.

Is there anything else i should replace while i am in the trans .

Everything else looks good.

I am going to include pics as soon as i learn how.
