The floor/defrost is a default setting when no vacuum is present to move the actuator.

The two common causes of this are the vac line from the manifold of the engine is disconnected or broken between the manifold and the vacuum canister (92-95 on firewall next to wiper motor, 96-99 under passenger front fender, 00+ unsure of location) or 96-99 the programmer nipples have collapsed.

The first is pretty straight forward and can be tested at the purple incoming hose at the HVAC programmer under the dashboard (96-99) and info on the second can be found by searching "Vents" by "James Tharp"
