steering wheel jerks while braking

Default steering wheel jerks while braking

I've searched for the past 30 minutes or so and could not find a topic on this. When I brake from anything over 40 or so my steering wheel jerks up and down, it also feels likes my antilock is always on because my brake pulsates while I brake no matter how hard I do it. If I almost slam on the brakes the steering wheel wont do it, it only does it under light to moderate braking. All of this is only noticeable to the person behind the wheel. TIA for any input to the problem/solution.

what is the condition of the rotors? maybe time for rotor/pads.

I'm not sure. I've only had the car since May of 2007. I plan to change them sometime next week while I'm on break. Any suggestions on what brand and type to get?
