Code 58:

Pass-Key II Fuel Enable Circuit Signal Lost While Engine Running

I've seen this before - but I don't know a common solution. It may be a loose connection in or around the ignition. Sometimes bad connections at the battery can cause it. Some of your symptoms sound like low voltage problems. This one needs a diagnostic following the dealer manual.

Your other problem could be the IAC valve. What you need to do is use a Tech 1 to command the IAC valve to open anc close and then you can see what happens to the engine. Have you had a tune-up lately? Cleaned the throttle body?
